AIAG & VDA FMEA changes

To support you in implementing AIAG & VDA FMEA, ITVC Global would like to send you a summary of 10 changes to AIAG & VDA FMEA as follows:


1. Give 7 steps to perform FMEA:

The FMEA implementation process has been revised and requires a seven-step approach:

• Step 1 | Planning and Preparation

• Step 2 | Structure Analysis

• Step 3 | Function Analysis

• Step 4 | Failure Model Analysis (Failure Analysis)

• Step 5 | Risk Analysis

• Step 6 | Optimization

• Step 7 | Results Documentation

These seven steps are organized into three stages:

• Steps 1 through 3 represent the Systems Analysis phase of a PFMEA study.

• Steps 4 to 6 represent the Failure Analysis and Risk Reduction phase of a PFMEA study.

• The third phase, implementing communication - is Step 7, Documenting results


2. Scope definition

The requirement clearly states what is and is not included in the FMEA


3. Add FMEA-MSR method

MSR: Monitoring and System Response

A new method has been added which is FMEA-MSR. FMEA-MSR is understood as a supplement to “FMEA for surveillance and response systems”. The FMEA-MSR is intended to maintain a safe state (i.e. ensure safety) or regulatory compliance (i.e. comply with environmental requirements) during the customer's operations.


4. Structural analysis

In the old PFMEA - only the stage step (name and brief description of the stage) was mentioned - but in the new PFMEA, the structural analysis step has been implemented - in which 3 items are analyzed: process, process. stages and stage elements. This helps us better understand the process under consideration.


5. Functional analysis

Functional requirements have been divided into three levels in the new FMEA – this helps to identify failures and their impact more clearly.

- Function of the process item / Function of the process. The function of system, subsystem, part element, or process

- Function of the process step and product characteristic - (quantitative value is optional / prioritize quantitative data)

- Function of the process work element and process characteristics / Function of the process work element and process characteristics


6. Modify the S,O, D scoring table

- Table S - Shows the impacts on 3 categories: Your Plant (Factory), Ship to Plant (Customer Factory), and Customer (Customer) to give severity score (S)

- Table O – Lists criteria on the likelihood of occurrence, process experience, and preventive controls that help determine the likelihood of a potential error occurring.

- Table D – Specify detection criteria to determine score D – detection ability easily.


7. Eliminate risk priority indicators (RPNs)

During Step 5 of the FMEA Process - Risk Analysis, was the RPN eliminated?

That's right, the Priority Risk Number (RPN) is replaced by the Action Priority (AP) which comes from a series of tables that identify APs based on a ranking of Severity, frequency of occurrence, and likelihood—detection ability of the latent error model. AP is not a risk priority but a priority for action (high, medium, or low) to minimize the risks of failures that cause functions to not operate as planned. While RPNs encourage ranking potential defects from high to low, AP tables determine absolute priority for action regardless of how many other items have been identified in the FMEA study.


8. Two types of actions are proposed

In step 6 - Optimization, the “recommended actions” section has been changed to two columns: Preventive actions and Detection actions.


9. Assign people to implement

In step 6 - Optimization, it is clear who is responsible for taking preventive actions and detecting actions for potential errors.


10. Documentation of results (PFMEA results documentation)

The new PFMEA requires documentation of FMEA development results for internal or external communication. Required documents may include:

- Execution summary

- Scope of FMEA

- S/O/D rating table

- Action Priority (AP)

- Results and conclusions of the analysis.
