Sketches may be attached where needed for illustration. A control plan does not replace the information in detailed operating instructions.

Control Plan document
The document provided is a completed control plan for an actual product, rather than a blank template.
The Control Plan covers the following 26 items:
1. Prototype / Pre-launch / Production: Identify the product development stage for which the control plan is prepared.
2. Control Plan number: Enter the control plan reference number. Page numbers may be included for a multi-page plan.
3. Part number / Latest change level – system, assembly or component identification: If applicable, enter the latest engineering change level.
4. Part name: Enter the name of the product.
5. Organization / Plant: Enter the company and plant or department preparing the control plan.
6. Organization / Supplier code: Enter the code assigned to the company by the customer.
7. Key contact / Telephone: Enter the name and telephone number of the responsible contact.
8. Core team: List the team members.
9. Organization approval / Date: Obtain the plant engineering approval signature if required.
10. Date (original): Enter the date the plan was first prepared.
11. Date (revision): Enter the date of the latest control plan update.
12. Customer engineering approval / Date: Obtain the approval signature of the customer’s engineering department.
13. Customer quality approval / Date: Obtain the approval signature of the customer’s quality department.
14. Other approval / Date: Record any other approvals.
15. Part / Process number: Use the reference from the process flow chart.
16. Process name / Operation description: Identify the operation and describe its activities accurately.
17. Machine, device, jig or tool: Identify the equipment, tools, jigs and other resources required at each manufacturing operation.
18. Number: Enter reference numbers from other documents, such as the process flow chart, FMEA and drawings.
19. Product: Identify the product characteristics to be controlled.
20. Process: Identify the process characteristics to be controlled.
21. Special characteristic classification: Use the symbols specified by the OEM to classify special characteristics.
22. Product / Process specification and tolerance: Specifications and tolerances may come from sources such as drawings, design reviews and product standards.
23. Evaluation / Measurement technique: Describe the measurement system used, which may include fixtures, gauges or measuring instruments.
24. Sample size / Frequency: When a sampling plan applies, specify the sample size and frequency.
25. Control method: Briefly describe how the operation is controlled, including relevant document references where appropriate. Select the control method according to the process classification. Operation controls may include SPC, inspection, attribute data, error prevention and sampling plans.
26. Reaction plan: Specify the corrective actions needed to prevent nonconforming products or an unstable process.
